儿童肥胖症亟需有效防控措施
肥胖症(Obesity)是机体脂肪聚积过多达到危害健康程度的一种慢性代谢性疾病。其可诱发多种严重损害人体健康的合并症如糖尿病,心血管疾病,高血压,代谢综合征等。2010年,全球因超重和肥胖死亡的人数高达340万,预测到2025年将增至780万[1]。随着生活方式等因素的改变,儿童肥胖症的发病率也越来越高。2013年,发达国家儿童青少年中有23.8%的男孩和22.6%的女孩为肥胖或超重[2]。据统计,2010年我国儿童肥胖患病率相比1985年增加了近50倍,且仍在持续增长[3]。Meta分析表明BMI较高的儿童罹患高血压、脂代谢紊乱及胰岛素抵抗等心血管及代谢性疾病的风险显著增高[4],前瞻性队列研究亦指出儿童肥胖可持续到青春期甚至成年[5-6]。因此,儿童肥胖症已成当今重要的公共卫生问题,而有效遏制儿童肥胖的快速增长,急需探究其影响因素,以便制订适合我国儿童肥胖的防控措施。
肠道菌群可能成为儿童肥胖治疗新靶点
肥胖是受环境和遗传因素共同调控的。除膳食、体力活动等之外,近年来虽然已有越来越多的研究显示肠道菌群亦与肥胖的发生密切相关[7],但关于肠道菌群谱与儿童肥胖的关系的研究却十分缺乏。就目前来看,既往研究多集中于Bacteroidetes、Firmicutes的研究,难以较全面反应肠道菌群与肥胖的关系,且研究结论不尽一致,肠道菌群究竟是否影响儿童肥胖的发生结论尚不肯定。而进一步探索肠道菌群对儿童肥胖的影响,将为我国儿童肥胖症的干预和治疗提供新靶点。
短链脂肪酸可能参与调节机体脂肪积累
目前,不仅肠道菌群对儿童肥胖的发生是否产生影响存在争议,肠道菌群如何调节机体脂肪积累的机制同样尚不明确,但越来越多的研究表明肠道菌群代谢产物短链脂肪酸(short chain fatty acid, SCFA)可能在其中扮演着至关重要的角色[8]。SCFA是膳食纤维通过肠道菌群代谢后产生的含碳2~4个的支链脂肪酸,如乙酸、丙酸及丁酸。SCFA在体内除了可作为机体的能量来源,还可作为信号分子调控相关通路对机体代谢发挥作用。此外,体外研究显示乙酸和丙酸可减少脂肪细胞对脂肪的聚积[9],小鼠模型表明注射乙酸可降低食欲[10],而给小鼠补充SCFA可抑制其体重增长[11-12]。另有Schwiertz等报道BMI处于超重及肥胖者其粪便总SCFA的含量却显著高于体重正常者[13]。这些研究均提示,肠道菌群可能通过SCFA发挥对宿主的脂肪积累的调节作用,但目前尚无研究探讨肠道菌群代谢产物SCFA是否影响儿童肥胖的发生,及其在肠道菌群影响儿童肥胖的病因链上所起的作用,所以肠道菌群是否通过影响SCFA的水平进而对儿童肥胖产生作用,仍待进一步研究。
综上所述,揭示肠道菌群对儿童肥胖的作用及其作用环节,丰富儿童肥胖症的环境危险因素及发病机制等理论知识,将会在实践上从肠道菌群角度为儿童肥胖症的早期干预和治疗提供新靶点。
